Physician Referrals Made Easy

Referring a patient to Oklahoma Pain Management is fast and simple.

We provide a referral PDF to assist your office, but only basic patient information is needed to get your patient scheduled.

Once we receive a referral:

  • We contact the patient within 24 hours
  • Appointments are scheduled within one week
  • Our team handles the rest

If your patient is in pain, call us – we make the referral process easy for both providers and patients.

The information we absolutely need before scheduling a patient is:

  • Demographic information including date of birth, age, gender, social security number, address and any phone numbers you have on record
  • Insurance information – preferably a copy of the insurance card.
  • Diagnostic studies including any and all MRI, CT, or any other records the patient may have if you are referring for a specific procedure. We can always do an evaluation without diagnostic imaging.
